This policy describes how LifeTrack Services, Inc. (“LifeTrack,” “we,” “us”) collects, uses, and shares information through TakeMySchoolSurvey.com (the “Service”). It applies only to TakeMySchoolSurvey.com, not to our other websites or products.
We provide survey tools to schools and related education programs. When a school uses the Service, that school decides what to ask, who may respond, and how results are used. We process that information to operate the Service for the school, including contacting respondents about later surveys when contact information was collected for that purpose.
1. Who this policy covers
This policy applies to:
- Survey respondents (usually students, and sometimes parents, staff, or alumni) who open a survey link and submit answers
- School staff and LifeTrack users who log in to create surveys, upload student lists, export results, or build reports
- Website visitors to TakeMySchoolSurvey.com
This policy covers only TakeMySchoolSurvey.com. Other LifeTrack sites, including Graduate-Surveys.com and AthleticSurveys.com, are separate products and are not covered here.
2. Our role and the school’s role
The school or program that runs a survey chooses the questions, who is invited, whether a password is required, and whether the survey is public, unlisted, or private. That organization is responsible for having a lawful basis to collect student information and for how it uses the results.
LifeTrack hosts the Service, stores the data the school puts into it, and makes that data available to authorized users of the school’s account. We do not sell student or survey data, and we do not use it for advertising.
3. Information we collect
Survey respondents
- Survey answers to whatever questions the school includes (for example opinions, ratings, demographics, or comments)
- Identity information the school uploads so a student can take a password-protected survey, which may include first name, last name, email address, student number, program, and a survey access password
- Contact information for follow-up surveys, such as an email address, when it is provided in a student list or entered in a survey. We may use that information later to invite the same person to another survey on TakeMySchoolSurvey.com
- Submission details such as which survey was taken and when the response was saved
If the school’s survey includes name or email fields, we may fill those fields from the student list the school uploaded. Public or unlisted surveys may be submitted by anyone with the link, and those answers are stored the same way.
School staff and other logged-in users
- First name, last name, and email address
- A hashed account password
- Surveys, templates, reports, and files they create or upload
- Student lists and exported response files they upload or download
Technical information
- Standard server and hosting logs, which may include IP address, browser type, date/time, and pages requested
- Cookies needed to keep staff logged in and to protect forms from cross-site request forgery
We do not use Google Analytics, advertising pixels, or similar marketing trackers on the Service.
4. How we use information
We use information to:
- Let respondents open and submit surveys
- Authenticate staff accounts and student survey access codes
- Store, display, export, and report survey results for the school
- Contact respondents about potential follow-up surveys on TakeMySchoolSurvey.com, using information such as email addresses collected for that purpose
- Operate, secure, back up, and troubleshoot the Service
- Communicate with schools about their accounts and support
- Comply with law and protect the Service against misuse
Follow-up contact is for later school surveys on this Service, not for advertising. We do not use student survey data to build marketing profiles, and we do not sell personal information.
5. How we share information
We share information only as needed to run the Service:
- With the school that owns the survey. Authorized staff can view, export, and delete student lists and responses for surveys they manage.
- With service providers that host the website, database, and backups, or that deliver page assets (see Cookies below). They process information on our instructions.
- If required by law or to protect the rights, safety, or security of students, schools, or LifeTrack.
- In a business transfer such as a merger or sale, subject to continuing privacy protections.
We do not share student or survey data with advertisers or data brokers.
6. Cookies and third-party services
The Service uses:
- Session cookies for logged-in staff (kept for up to 30 days) so they stay signed in
- A CSRF security cookie so staff actions come from the real site, not a forged request
Survey takers are not required to create an account. Staff login cookies are not used to advertise to students.
Some pages load fonts, icons, or scripts from third-party content networks (for example jsDelivr, Cloudflare/cdnjs, UNPKG, Google Fonts, or DataTables). Those providers may receive the visitor’s IP address and browser information when the page loads. We use them to display the site, not to run ads.
7. Students and children
The Service is used by K–12 schools, so respondents may include children under 13. We collect student information only because a school (or similar education program) asked us to as part of that school’s survey.
Schools are responsible for deciding whether a survey is appropriate for their students, what questions to ask, and whether parent notice or consent is required. Parents or guardians who have questions about a specific survey should start with their school.
We do not use children’s information for advertising. Email addresses and similar contact details may still be used to invite a student to a later survey on TakeMySchoolSurvey.com when that information was collected for follow-up. We do not condition participation on providing more personal information than the school’s survey requires.
8. Education records
Information a school stores in the Service (such as student names, student numbers, and survey responses) may be an education record under FERPA or similar state student-privacy laws. We handle that information as a service provider to the school. We may also use contact information from the Service to invite respondents to follow-up surveys on TakeMySchoolSurvey.com. We do not use education records for advertising or other unrelated purposes.
Requests to inspect, correct, or delete a student’s education record should generally be made to the school. We will assist the school with access, correction, or deletion in the Service when the school asks us to.
9. How long we keep information
We do not have a fixed retention period. Survey responses, student lists, email addresses collected for follow-up, and staff accounts are kept until they are deleted from the Service.
School staff can export or clear responses, archive surveys, and permanently delete surveys from their account tools. We may also delete information when a school asks us to. Database backups may still contain deleted information until those backups are replaced.
Because there is no automatic deletion schedule, information used for follow-up surveys may be kept indefinitely unless the school or LifeTrack removes it.
10. Security
We use reasonable administrative and technical safeguards to protect information on TakeMySchoolSurvey.com. No online service is completely secure, and these measures reduce risk; they do not eliminate it.
Accounts and access
- Staff tools (creating surveys, uploading student lists, exporting responses, and building reports) require a logged-in account
- Staff account passwords are stored as one-way hashes, not as readable passwords
- After login, a session cookie keeps staff signed in for up to 30 days. That cookie is marked HttpOnly and, when the site is served over HTTPS, Secure
- Staff actions are protected against cross-site request forgery (CSRF)
- Surveys may be private, unlisted, or public. A school can also require a survey access password. Public or unlisted surveys can be opened by anyone who has the link
How data is stored and sent
- The Service is designed to be used over HTTPS so information in transit is encrypted when the site is served that way
- Survey answers, student lists, and related records are stored in a database on our hosting environment, with backups of that database
- We use prepared database queries and standard browser security headers (such as Content Security Policy and clickjacking protections) on administrative tools
Who can see the data
- Authorized school staff and LifeTrack personnel who need access to operate or support the Service can view student lists, email addresses, survey responses, and exports
- We do not make student or survey data public except to the extent a school publishes a public or unlisted survey link
Schools should use strong unique passwords, give login access only to people who need it, treat exported CSV files as confidential, and avoid putting highly sensitive information in a survey unless they have a clear need. If we become aware of a security incident that affects personal information, we will take steps to contain it and notify affected schools as appropriate.
11. Your choices and rights
Students, parents, and guardians. Contact your school first about a survey, your answers, or a student list. The school can ask us to correct or delete data in the Service.
School staff. You can update your account information through LifeTrack support. You can log out to end your session. You can export or delete survey data you are authorized to manage.
California and similar state laws. We do not sell personal information and we do not share it for cross-context behavioral advertising. If the information belongs to a school survey, we may need to route the request through that school.
12. Changes
We may update this policy as the Service or the law changes. The effective date at the top will be revised when we do. Continued use of the Service after an update means the revised policy applies.
